Court-Accepted Substance Abuse Evaluations for Families
Our Custody / Family Court SUD Assessments are designed to evaluate substance use concerns in relation to parenting, family functioning, recovery, and overall stability. We work with respondents, attorneys, CPS, probation, and other involved providers to gather the information necessary to complete a comprehensive clinical evaluation.
What to Expect
Our assessment process may include:
- Clinical interview and screening
- Review of court documents and collateral information
- Substance use history and treatment history review
- Review of criminal, driving, or CPS history when applicable
- Coordination with attorneys, providers, and other approved collateral contacts
- Written clinical recommendations and treatment recommendations when indicated
We strive to provide a respectful and nonjudgmental environment while maintaining clinical objectivity and ethical standards throughout the assessment process.
Before You Begin
Important Information
The assessment process relies on timely receipt of required documentation, signed ROIs, collateral information, and requested records. Missing information may delay scheduling or completion of the assessment and could result in additional administrative or coordination fees.
In situations where required information is not provided, ROIs are revoked, or the assessment cannot be completed according to clinical, ethical, or legal standards, the assessment process may be suspended, closed, or considered incomplete. In some cases, a new assessment and repayment of assessment fees may be required.
Our clinicians reserve the right to determine whether sufficient information is available to provide an accurate and clinically appropriate assessment and recommendation.
Assessment Fee
- Custody / Family Court SUD Assessments: $825
- Payment is required in full prior to scheduling.
Required Releases of Information (ROIs)
To ensure a thorough and timely assessment, signed Releases of Information may be required for:
- Emergency Contact
- Respondent’s Attorney
- Petitioner’s Attorney
- Additional Collateral Contacts
If applicable:
- CPS Case Worker
- Probation Officer
- Employer
- SUD, Mental Health, Medication Management, or Medical Providers
Required Documentation
Please provide copies of the following documents prior to or at the time of the assessment:
- Criminal History / Defendant Case History
- Full Driving Abstract
- Parenting Plan and/or Proposed Parenting Plan
- Declarations from both parties
- Allegations or court filings submitted to the court
Additional records or documentation may be requested if clinically relevant to the assessment process.
